The differentiation of arterial versus venous occlusion in free tissue transfers has rarely been described. This study investigated changes in blood flow caused by arterial and venous occlusion and the potential for laser Doppler flowmetry to distinguish between these 2 conditions for better clinical assessment and management of free tissue transfer.

Our data reflect the experience with LDF for investigating the hemodynamic status of free flaps and their arteries and veins. Despite the limitations, LDF could demonstrate the potential to differentiate between arterial and venous vessel occlusion. The LDF can be a tool to improve the salvage in free tissue transfers. Further studies with a larger cohort are needed to objectively compare the various monitoring devices for its applicability, reliability, and clinically relevant parameters.

The primary limitations of this pilot study were the low number of cases and that only clinical monitoring was performed postoperatively.
