To explore alterations in regional and network-level neural function and to determine the association between these alterations in intrinsic neural activity and symptom severity in OCD.

The findings demonstrated that patients with OCD had abnormal regional brain function and aberrant FC in fronto-striatal-thalamic circuitries. Moreover, the FC in striato-thalamic junction was negatively correlated with the Y-BOCS total score, which indicated a connectivity-based pathophysiological process in OCD. Beyond the classical fronto-striatal-thalamic circuitries, the SMA and temporal lobe are also involved in the pathology of OCD, which provides support for further efforts to explore the complex functional alterations of OCD.

1. The study only included patients with OCD who did not meet diagnostic criteria for other disorders, which may decrease the generalizability of the findings. 2. The study did not evaluate the neuropsychology to assess cognitive function. 3. The study is a cross-sectional study. A follow-up study using rs-fMRI is needed to investigate altered regional activity and FC after therapeutic intervention and symptom improvement. 4. The FC alterations between OCD patients and healthy controls and the whole-brain correlations were not significant when a relatively conservative correction approach (such as FWE) was used.
1:Experimental Design and Method Selection:
The study used resting-state functional magnetic resonance imaging (rs-fMRI) to examine 68 drug-naive OCD patients and 68 healthy control subjects (HCS). Regional cerebral function was evaluated by measuring the fraction of amplitude of low-frequency fluctuation (fALFF). Regions with fALFF alterations were used as seeds in whole-brain functional connectivity (FC) analysis.

A total of 68 drug-naive OCD patients and 68 sex-, age-, and education-matched HCS were recruited.
3:List of Experimental Equipment and Materials:
The MRI examinations were performed on a 3-Tesla MRI system (EXCITE, General Electric, Milwaukee, WI) with an 8-channel phased-array head coil.
4:Experimental Procedures and Operational Workflow:
Resting-state fMRI sensitized to changes in BOLD signal levels of the whole brain was performed via a gradient-echo echo-planar imaging (EPI) sequence.
5:Data Analysis Methods:
Statistical analyses of fALFF and FC differences between OCD patients with HCS were performed voxel-by-voxel using a two-sample t-test in Statistical Parametric Mapping version 8 (SPM8). Whole brain correlation analyses were performed to identify the association between functional neural correlates and OCD symptom severity evaluated using Yale-Brown Obsessive Compulsive scale (Y-BOCS) and subscale scores.
