To evaluate whether tissue oxygenation index (TOI) could be associated with return of spontaneous circulation (ROSC) and used to help guide the decision to either terminate cardiopulmonary resuscitation (CPR) or proceed to extracorporeal CPR (ECPR).

Initial TOI is an important correlate of ROSC and it increases the accuracy of prediction of ROSC significantly when Utstein variables are used in the prediction model. Initial TOI values, based on specific cut-off values, also showed some potential for decision-making on CPR termination ('TOR zone') or ECPR initiation ('ECPR zone').

Single-centre study with a small sample size. The validity of TOI cut-off values has not been evaluated. NIRS technology is still developing and there is no uniform algorithm among all NIRS device manufacturers.
1:Experimental Design and Method Selection:
Observational study assessing patients with out-of-hospital cardiac arrest (OHCA) with non-traumatic cause receiving CPR on arrival at the ED between 2013 and
2:TOI monitoring was discontinued either on CPR termination after ROSC was reached or on patient death. Patients were classified into two groups:
20 ROSC and non-ROSC group.
3:Sample Selection and Data Sources:
Patients with OHCA admitted to the ED who were ≥18 years old with non-traumatic cardiac arrest were included. Exclusion criteria were patients with hypothermia and patients who had achieved ROSC before arrival at ED.
4:List of Experimental Equipment and Materials:
NIRO-200NX (Hamamatsu Photonics) for measuring cerebral tissue oxygen saturation (TOI).
5:Experimental Procedures and Operational Workflow:
Bilateral forehead probes were placed immediately after arrival to the ED. TOI recording started soon after the probes were placed and ended either on CPR termination once ROSC had been achieved or when the patient died.
6:Data Analysis Methods:
Continuous variables were summarised as the median (IQR) or mean (SD) according to the distribution using the Shapiro-Wilk test. Categorical variables were compared with the Χ2 test. Multiple logistic regression analysis was used to assess the relationship between ROSC and initial TOI with core Utstein variables.
