To investigate the diagnostic accuracy of BI-RADS descriptors in predicting malignancy for additional non-mass enhancement (NME) lesions detected on preoperative 3T dynamic contrast enhanced MRI (DCE-MRI) in patients with newly diagnosed breast cancer.

Malignancy is strongly indicated when additional NME lesions show linear or segmental enhancement on preoperative 3T DCE-MRI in patients with recently diagnosed breast cancer. Percutaneous biopsy or wide surgical excision should be considered in these cases.

The study was retrospective, and there was a large size difference between malignant and benign lesions which may cause statistical selection bias. Most benign lesions were not pathologically diagnosed (though stable or disappeared on follow-up). Kinetic features of NME lesions were not evaluated.

4. Experimental Procedures and Operational Workflow: MRI was performed with patients in prone position. T1- and T2-weighted images and dynamic contrast-enhanced sequences were acquired. Gadoterate meglumine was injected at 2 mL/s. Post-processing included subtraction images, time-intensity curves, and maximum intensity projection images.

Statistical analysis was performed using SPSS version 20.0. Differences in MRI features between benign and malignant lesions were assessed using Fisher's exact test or chi-square test. Sensitivity, specificity, PPV, NPV, and logistic regression were calculated.
