D30Angiogenesis evaluation in locally advanced colo-rectal and gastric cancers by probe-based Confocal Laser Endomicroscopy (pCLE)
摘要: Probe-based Confocal Laser Endomicroscopy ( pCLE) is an innovative endoscopic technique that allows taking high resolution images of the mucosa, facilitating the identification of cellular and subcellular microstructures permitting an evaluation of the microvasculature during endoscopic examination. Angiogenesis is a hallmark of cancer development inducing the formation of new vasculature to support its growth. The aim of our study was to evaluate tumor neoangiogenesis through pCLE imaging in locally advanced gastric and rectal cancer patients, before and after neoadjuvant radio-chemotherapy (RT/CT). Methods: 74 consecutive patients affected by Rectal Cancer (RC, 19F, 55 M mean age: 65 years) and 28 consecutive patients with Gastric Cancer (GC, 7F, 21 M mean age: 64 years) underwent endoscopy with pCLE-GastroFlex UHD probe (Mauna Kea Technologies) and i.v. fluorescein infusion in order to evaluate intratumoral vascularization and to evaluate the efficiency of blood flow. After RT/CT treatment, 33 RC (27M, 6F) and 7 GC (2F, 5M) patients were revaluated using pCLE; neoangiogenesis was evaluated according to Cannizzaro-Spessotto (CS) scale, assigning one point to each of the following features: tortuous vessels, large vessels, leakage and defective flux. Results: 22 out of 35 (62.8%) RC and 4 out of 9 (44,4%) GC patients showed an improvement of angiogenesis index, while in the remaining 13 RC (39.4%) and 5 GC (71.4%) there were no changes of the vascular architecture following the treatment. There was a significant difference ( p < 0.05) in neoangiogenesis CS scores between RC pre- (median CS score: 2.7) and post-RT/CT (median CS score: 1.6), while there’s no difference ( p > 0.05) between GC patients pre- (median CS score: 2.6) and post-therapy (median CS score: 2.0). Our data show a better reactivation of vessels’ morphology and functions in RC patients, with an improvement of angiogenesis index. In GC patients median angiogenesis index remained unmodified, without positive changes in vascular morphology, probably due to the presence of fibrosis. Conclusions: The results of our work demonstrate that pCLE technique is suitable to evaluate the alterations of the intratumoral microvasculature and reveal a functional improvement of vasculature in post-therapy RC patients. It may constitute a innovative approach in order to identify subjects that respond to the therapy, improving the outcome of the patients.

摘要: Objective: the purpose of this review is to present the different randomized clinical protocols using low level laser in the management of oral mucositis, in the last eighteen years of study. Literature review: oral mucositis remains a major problem to be solved in clinical oncology. This cytotoxic effect limits food intake by mouth due to the pain and discomfort in chewing and/or swallowing. Nutritional status may be aggravated contributing to increased morbidity and hospital stay, and providing negative impact on the quality of life of cancer patients, and on their treatment. Disturbances in the integrity and function of the oral cavity are due to the fact that radiotherapy and chemotherapy are not able to destroy tumor cells without damaging normal cells. Numerous treatments are described in literature and there are several methodologies for the treatment of oral mucositis, yet there is no consensus on the best strategy for prevention and therapy. Final considerations: several studies have shown good results with the use of low level laser, which main indications include anti-inflammatory and analgesic effects, and inducing tissue repair. However, there is no definition of a single protocol for this promising resource so far to be applied in the therapy of this pathology.

Optical examination of the efficacy of contact needle therapy for chemotherapy-induced peripheral neuropathy: integration of inspection in Kampo therapy with color spectrum information
摘要: Cancer chemotherapy-induced peripheral neuropathy (CIPN) deteriorates the patient’s quality of life, often resulting in discontinuation of treatment with potentially effective chemotherapy. Contact needle therapy (CNT) is one of the traditional Japanese acupuncture methods based on the concept of Kampo (Japanese traditional) medicine. CNT on CIPN has been considered effective empirically, but an objective evaluation has proven difficult. We have focused on the oxygen saturation besides the symptom score. We have already proposed optical path-length matrix method (OPLM) for estimating oxygen saturation of the skin, which may help in locating peripheral tissue damage, including neuronal damage. Four patients with CIPN were treated with CNT. The severity of CIPN was evaluated using the Common Terminology Criteria for Adverse Events (CTCAE) ver. 4 and FACT/GOG-Ntx before and after CNT. We also measured oxygen saturation and hemoglobin concentration at the right toe-tip. Correlations among the values were calculated as correlation coefficients. There were significantly improvements in FACT/GOG-Ntx and hemoglobin post-CNT than in pre-CNT (p < 0.05). The oxygen saturation showed broad correlation (0.5 < |r| < 1) with hemoglobin. The oxygen saturation of skin reflects tissue injury and blood stagnation status, and can help in objective evaluation by measuring skin color spectrum. CNT might be considered as one of the safe and effective alternative therapeutic options for CIPN. Our method of evaluation using skin color spectrum information integrates, in a sense, the inspection in Kampo (Japanese traditional medicine) therapy and scientific methodology.

A Case of Advanced Malignant Pleural Mesothelioma Treatment with Chemotherapy and Photodynamic Therapy
摘要: Malignant pleural mesothelioma (MPM) is an aggressive, treatment-resistant, and generally fatal disease. A 68-year-old male who was diagnosed with MPM at another hospital came to our hospital with dyspnea. We advised him to take combination chemotherapy but he refused to take the treatment. That was because he had already received chemotherapy with supportive care at another hospital but his condition worsened. Thus, we recommended photodynamic therapy (PDT) to deal with the dyspnea and MPM. After PDT, the dyspnea improved and the patient then decided to take the combination chemotherapy. Our patient received chemotherapy using pemetrexed/cisplatin. Afterwards, he received a single PDT treatment and then later took chemotherapy using gemcitabine/cisplatin. The patient showed a survival time of 27 months, which is longer than median survival time in advanced MPM patients. Further research and clinical trials are needed to demonstrate any synergistic effect between the combination chemotherapy and PDT.

Physicochemical and antimicrobial photodynamic chemotherapy (against E. Coli) by indium phthalocyanines in the presence of silver-iron bimetallic nanoparticles
摘要: In this work, Schiff base indium phthalocyanines: In-Cl tetra N,N-dimethyl-4-(methylimino) phthalocyanine (complex 1b) and In-Cl tetra-phenoxy N,N-dimethyl-4-(methylimino) phthalocyanine (complex 2b) substituted at the peripheral positions with 4-dimethyl benzaldehyde are synthesized from tetra amino and tetra phenoxy amino phthalocyanines, respectively. These complexes were further quartenised with 1,3-propanesultone to form zwitterionic complexes 1 and 2, respectively. Silver-iron dimers (Ag-Fe3O4) and silver-iron core shell (Ag@Fe3O4) nanoparticles (NPs) were linked to the synthesised complexes. The photophysical and photochemical behaviour of the complexes and their conjugates with NPs were investigated in dimethyl sulfoxide. Complexes 2 and 2b and their conjugates were then used for photodynamic antimicrobial chemotherapy on E. coli. The zwitter ionic photosensitiser 2 and its conjugates showed better efficiency for photodynamic antimicrobial chemotherapy compared to their neutral counterparts.

Early Therapeutic Prediction Based on Tumor Hemodynamic Response Imaging: Clinical Studies in Breast Cancer with Time-Resolved Diffuse Optical Spectroscopy
摘要: This study reports data from three clinical studies using the time-resolved diffuse optical spectroscopy (TRS) system among breast cancer patients. The parameters of oxy-hemoglobin (O2Hb), deoxy-hemoglobin (HHb), total hemoglobin (tHb), and oxygen saturation (SO2) were evaluated using TRS, and its efficacy was tested in three trials. In trial 1, we recruited 118 patients with primary breast cancer to estimate the tumor detection rate. The cumulative detection rate was 62.7%, while that in T stage 0 was 31.3% and in T stage 1 was 44.7%. These were lower than those of T stage 2 (78.9%) and T stage 3 (100%). Next, we used TRS to monitor tumor hemodynamic response to neoadjuvant chemotherapy (n = 100) and found that pathological complete response (pCR) tumors had significantly lower tumor tHb than non-pCR tumors; a similar result was observed in estrogen receptor (ER)-negative tumors, but not in ER-positive tumors. The third trial monitored hemodynamic response to antiangiogenic therapy, bevacizumab (n = 28), and we demonstrated that sequential optical measurement of tumor SO2 might be useful for detecting acute hypoxia 1–3 days after bevacizumab initiation. Next, response monitoring of neoadjuvant endocrine therapy (n = 30) suggested that changes in tumor tHb during treatment can predict and distinguish between responsive and non-responsive tumors early in letrozole therapy. In conclusion, our results show that hemodynamic monitoring of tumors by TRS could pair the unique features of tumor physiology to drug therapy and contribute to patient-tailored medicine. We recently established a platform for performing TRS in patients with breast cancer.

β-Cyclodextrin–Hyaluronic Acid Polymer Functionalized Magnetic Graphene Oxide Nanocomposites for Targeted Photo-Chemotherapy of Tumor Cells
摘要: A multifunctional targeted drug delivery platform (CDHA–MGO) has been successfully constructed by grafting β-cyclodextrin–hyaluronic acid polymers (CDHA) to Fe3O4–graphene oxide (MGO). The obtained CDHA–MGO nanocomposite has good water-dispersibility, easy magnetic separation, high near-infrared (NIR) photothermal heating, and excellent biocompatibility. The β-cyclodextrin-hyaluronic acid polymers ef?caciously enhance the doxorubicin (DOX) loading amount up to 485.43 mg·g?1. Meanwhile, the Fe3O4–graphene oxide provides a facile photothermal response mechanism to handle the NIR-triggered release of DOX in weak acidic solvent environments. Signi?cantly, the DOX-loaded nanocomposite (DOX@CDHA–MGO) has displayed CD44 receptor-mediated active targeting recognition and chemo-photothermal synergistic therapy of hepatoma cells. These ?ndings suggest that the as-prepared drug delivery platform would be of valuable potential for cancer-targeted photo-chemotherapy.

Flower-like Fe7S8/Bi2S3 Superstructures with Improved Near-Infrared Absorption for Efficient Chemo-Photothermal Therapy
摘要: Although various photothermal therapy (PTT) nanoagents have been developed in recent years, the rational design and easy synthesis of a PTT nanoplatform with improved near-infrared (NIR) absorption remains challenging. Herein, via a facile one-pot solvothermal strategy, hydrophilic nanosheet-assembled flower-like Fe7S8/Bi2S3 superstructures were fabricated successfully. Such nanoflowers exhibit improved NIR absorption, which is 1.54 times higher than that of pure Bi2S3 nanosheets at a wavelength of 808 nm. Attractively, these nanoflowers could be served as a drug delivery carrier with controlled release under pH/NIR stimuli, and display fascinating chemo-photothermal synergetic therapeutic effect both in vitro and in vivo. The resulting nanoflowers may open up a way for the design of other nanoagents with improved NIR absorption as well as chemo-photothermal cancer therapy effect.

Multiparametric monitoring of chemotherapy treatment response in locally advanced breast cancer using quantitative ultrasound and diffuse optical spectroscopy
摘要: Purpose: This study evaluated pathological response to neoadjuvant chemotherapy using quantitative ultrasound (QUS) and diffuse optical spectroscopy imaging (DOSI) biomarkers in locally advanced breast cancer (LABC). Materials and Methods: The institution’s ethics review board approved this study. Subjects (n = 22) gave written informed consent prior to participating. US and DOSI data were acquired, relative to the start of neoadjuvant chemotherapy, at weeks 0, 1, 4, 8 and preoperatively. QUS parameters including the mid-band fit (MBF), 0-MHz intercept (SI), and the spectral slope (SS) were determined from tumor ultrasound data using spectral analysis. In the same patients, DOSI was used to measure parameters relating to tumor hemoglobin and composition. Discriminant analysis and receiver-operating characteristic (ROC) analysis was used to classify clinical and pathological response during treatment and to estimate the area under the curve (AUC). Additionally, multivariate analysis was carried out for pairwise QUS/DOSI parameter combinations using a logistic regression model. Results: Individual QUS and DOSI parameters, including the (SI), oxy-hemoglobin (HbO2), and total hemoglobin (HbT) were significant markers for response after one week of treatment (p < 0.01). Multivariate (pairwise) combinations increased the sensitivity, specificity and AUC at this time; the SI + HbO2 showed a sensitivity/specificity of 100%, and an AUC of 1.0. Conclusions: QUS and DOSI demonstrated potential as coincident markers for treatment response and may potentially facilitate response-guided therapies. Multivariate QUS and DOSI parameters increased the sensitivity and specificity of classifying LABC patients as early as one week after treatment.

Photoactivatable Prodrug-Backboned Polymeric Nanoparticles for Efficient Light-Controlled Gene Delivery and Synergistic Treatment of Platinum-Resistant Ovarian Cancer
摘要: Combination of chemotherapy and gene therapy provides an effective strategy for cancer treatment. However, lacking of suitable co-delivery systems with efficient endo/lysosomal escape and controllable drug release/gene unpacking is the major bottleneck for maximizing the combinational therapeutic efficacy. Herein, we developed a photoactivatable Pt(IV) prodrug-backboned polymeric nanoparticles system (CNPPtCP/si(c-fos)) for light-controlled si(c-fos) delivery and synergistic photoactivated chemotherapy (PACT) and RNAi on platinum-resistant ovarian cancer (PROC). Upon blue light irradiation (430 nm), CNPPtCP/si(c-fos) could generate oxygen-independent N3? with mild oxidation energy for efficient endo/lysosomal escape through N3?-assisted photochemical internalization with less gene deactivation. Thereafter, along with Pt(IV) prodrug activation, CNPPtCP/si(c-fos) would be disassociated to release active Pt(II) and unpack si(c-fos) simultaneously. Both in vitro and in vivo results demonstrated that CNPPtCP/si(c-fos) displayed excellent synergistic therapeutic efficacy on PROC with low toxicity. This PACT prodrug-backboned polymeric nanoplatform may provide a promising gene/drug co-delivery tactics for treatment of various hard-to-tackle cancers.
