Ocular Aberrations and Corneal Thickness-Intraocular Pressure Relationship One Year after Laser in situ Keratomileusis (LASIK) Enhancement with An Aspheric Ablation Profile
摘要: Objective: To evaluate the effect of an aspheric LASIK refractive enhancement performed by relifting the original flap on the higher order aberrations (HOA) of the eye and the relationship between central corneal thickness (CCT) and intra-ocular pressure (IOP). Methods: Consecutive case series of 30 eyes requiring refractive enhancement from 1804 uncomplicated primary LASIK procedures performed in 2012. Flap relift and wavefront optimized ablation (Wavelight Allegretto Eye Q 400 Hz excimer laser) were performed in all cases. Pre-and post-operative examination included Shack-Hartmann aberrometry for 3 mm and 5 mm pupil sizes, CCT and IOP. Postoperative results at 1 year were compared to baseline values. Results: Changes in average values of coma, spherical aberration (SA), trefoil, CCT and IOP were not significant. Linear regression revealed significant associations between change (y) in trefoil and pre-enhancement value of trefoil for 3 mm pupil (0.828x-0.045, r=0.722, p<0.001), coma and pre-enhancement value of coma for 5 mm pupil (y=0.281x-0.030, r=0.501, n=30, p=0.048), SA for 3 mm pupil compared with change in CT (y=0.0080-0.0009x, r=-0.378, p=0.0392), and SA for 5 mm pupil compared with change in CCT (y=0.0035x-0.0541, r=0.524, p=0.0029). Also there was a significant association between IOP and CCT both pre-(IOP=0.0313CT-3.3, r=0.740, p<0.0001) and post-enhancement (IOP=0.0243CT-0.018, r=0.675, p<0.0001). Conclusion: LASIK enhancement with an aspheric ablation profile did not significantly impact on the average values for HOAs, IOP or CCT. For individual cases, the likely shift in magnitude of coma, trefoil and SA can be estimated and, the change in SA is correlated with changes in CCT. The change in the abscissa value linking IOP with CCT may be an indication of changes in biomechanical properties of the cornea in the central region.

Micro/nano-suction cup structure of silicone rubber fabricated by ArF excimer laser
摘要: Micro/nano-suction cups of silicone rubber, cylindrical swelling structures in micron size with a hole of hundreds nanometer diameter each, were fabricated by 193-nm ArF excimer laser-induced photodissociation of silicone rubber. To be periodically, silica glass microspheres of 2.5 μm diameter were aligned on silicone rubber via Al thin film during laser irradiation. The Al thin film underneath each microsphere was locally laser-ablated to enable a circular irradiation of subsequent laser pulses, then the exposed silicone rubber was photochemically swelled by the photodissociation of Si–O bonds of silicone rubber. Also, debris of aluminum surrounding the laser-ablated areas resulted in pushing microsphere up to adjust a focal point to the surface of silicone rubber to form a hole centered at each the swelled silicone. The fabricated structure showed clear superhydrophobic properties. Dependence of single pulse fluence of ArF excimer laser on the fabrication of micro/nano-suction cup structure was discussed, together with the variation of thickness of Al thin film from 10 to 100 nm. The use of the superhydrophobic silicone rubber we fabricated is expected as a kind of delivery system, toward holding a small object even in water by each micro/nano-suction cup structure.

Synthesis of Lithography Free Micro-Nano Electron Field Emitters Using Pulsed KrF Laser Assisted Metal Induced Crystallization of Thin Silicon Films
摘要: Hydrogenated amorphous thin silicon films (a-Si:H) deposited on metal coated glass substrates were investigated to analyze the effect of a novel processing technique called Laser Assisted Metal Induced Crystallization (LAMIC) on their electron field emission (FE) properties. Post-surface characterization of the processed films showed increased surface roughness and the presence of uniformly spaced “island-like” micro-nano structures on the surface of metal coated backplane samples. Best FE results were obtained from samples sputtered with a thin layer of Aluminum (Al) on top and cross laser annealed at 190 mJ/cm2 (y-axis) and 100 mJ/cm2 (x-axis). FE measurements indicate a low turn-on electric field of less than 16 V/mm with emission currents in the order of 10?6 A. FE results were found to be particularly dependent on the laser fluence and the surface morphology exhibited very high discharge resistance. Oxidation of the films was observed to deteriorate their FE characteristics, thereby increasing the emission threshold to 36 V/mm. Diode configured field emission display prototypes are fabricated to exemplify their potential as cold cathode emitters.

Excimer laser in percutaneous coronary intervention of device uncrossable chronic total and functional occlusions
摘要: Device uncrossable lesions are a challenge in interventional cardiology and despite improvements in balloon and microcatheter profile, rotational atherectomy is necessary in some circumstances in order to ablate and traverse the lesion. Nevertheless, the application of rotational atherectomy requires utilization of a specific wire, Rotawire, which is not always so easily navigable. Debulking of device uncrossable lesions can be performed by coronary laser over any kind of angioplasty guidewire. We present a series of six patients with chronic total (100%) and functional occlusions (99% stenosis) in whom after successful wire crossing, microcatheter failed to traverse the lesion. After coronary laser performance, 5 out of 6 lesions were successfully dilated, achieving a successful final angiographic result of 83.3%.

Meta-analysis of 308-nm excimer laser therapy for alopecia areata
摘要: Background: Alopecia areata (AA) is an autoimmune disease that can result in spontaneous hair loss. Currently, there is no US Food and Drug Administration (FDA) approved treatment, however new treatments are being investigated. Excimer laser and excimer lamp treatment have been suggested and have the benefit of mild/few adverse effects. Methods: A literature search and meta-analysis was performed to investigate the efficacy of the excimer laser and lamp for treatment of AA. Results: No controlled trials were found which utilized the 308-nm excimer lamp. Four controlled trials (N ? 105) testing the efficacy of the 308-nm excimer laser were identified. When laser treatment was compared to control measured through the number of responders to treatment, the standardized mean difference was 18.37 (95% CI: 3.28, 102.77) in favor of treatment (p < .0009, I2 ? 36%). Conclusion: Our results suggest that use of the 308-nm excimer laser can be effective in AA therapy however more studies are required observing both the 308-nm excimer laser and lamp.

The effects of 308-nm excimer laser on the infiltration of CD4+, CD8+ T-cells and regulatory T cells in the lesional skin of patients at active and stable stages of nonsegmental vitiligo
摘要: Background: To explore the associations between this treatment and CD4+, CD8+ T-cells and regulatory T cells (Treg). Methods: We collected the skin biopsies from 295 patients with nonsegmental vitiligo treated with or without 308-nm excimer laser. We revealed that patients at stable stage showed a better response to 308-nm excimer laser than those at active stage. Results: In comparison with untreated patients, CD4+ and CD8+ T cells were both reduced while Foxp3+ Treg cells, TGF‐ β and IL-10 were elevated in the lesional sites of patients who showed effective reaction to the treatment. In the treated patients at active stage, the infiltration of CD4+ and CD8+ T cells were significantly declined but Foxp3+ Treg cells, TGF‐ β and IL-10 were increased compared to those in untreated patients at active stage. These changes of cell infiltration were more obvious in the treated patients at stable stage, explaining why there were more patients at stable stage response better than patients at active stage. Conclusion: 308-nm excimer laser is effective to reduce the infiltration of CD4+ and CD8+ T cells but promote the infiltration Treg cells and secretion of TGF‐ β and IL-10 in the lesion skin of vitiligo patients, especially at stable stage.

Transvenous excimer laser-assisted lead extraction of cardiac implantable electrical devices in the Japanese elderly population
摘要: Background: The feasibility and safety of lead extraction of cardiac implantable electronic devices (CIEDs) in the elderly Asian population remain uncertain. We report the outcome and safety of transvenous excimer laser-assisted lead extraction of CIEDs in Japanese patients aged ≥80 years. Methods: Consecutive 235 patients (age 67 ± 14.9 years, 167 male) undergoing lead extraction of CIED with an excimer laser system (Philips, Andover, MA, USA) were included. Results: Of 235 consecutive patients, 51 (22%) were ≥80 years (age 86 ± 5 years, 14 were aged ≥90 years; 42 had pacemakers, 3 had implantable cardioverter defibrillators, and 6 had cardiac resynchronization therapy devices). The median implant duration was 110 ± 95 months. Patients aged ≥80 years had lower body mass index (BMI, 20.7 ± 3.1 kg/m2 vs. 22.6 ± 3.4 kg/m2, p < 0.001), higher incidence of CIED infection (98% vs. 63%, p < 0.001), and longer duration of hospitalization (31 ± 31 days vs. 24 ± 17 days, p = 0.010) than patients aged <80 years. There were no differences between the age groups in the rate of complete retrieval (94.1% vs. 95.1%, p = 0.726) and major complications, including cardiac tamponade and cardiogenic shock (2.0% vs. 2.7%, p = 1.000). Conclusions: Excimer laser-assisted lead extraction was safe and feasible in the Japanese elderly population with low BMI, despite the high likelihood of procedural complications. Early removal of infected CIEDs should be performed without delay in elderly Asian populations.

High-Concentration, Low-Temperature, and Low-Cost Excimer Laser Doping for 4H-SiC Power Device Fabrication
摘要: We developed a novel KrF excimer laser doping system for 4H-SiC power devices, and demonstrated laser doping of SiC with Al thin film deposited on the surface. As seen from the results of the Al depth profile, high concentration implantation (~ 1021 cm-3 at the surface) of Al was achieved by laser ablation of the Al thin film. A high, built-in-potential (~3.5 V) of the pn junction diode was clearly seen in the I-V curve. In addition, the contact resistivity of the deposited Al/Ti electrodes on the surface was 1.9 × 10?4 Ωcm2 by TLM (Transmission Line Model). It was confirmed that a high concentration of Al doping and low contact resistivity were achieved by the KrF excimer laser doping system.

Use of 3-Dimensional Imaging in Submental Fat Reduction Following Hyperthermic Laser Lipolysis
摘要: In a randomized controlled study, patients with a wide variety of hypopigmented scars, and striae alba had mean final pigment correction rates of approximately 100% by colorimetric analysis after 9 treatments of 308-nm excimer laser biweekly. However, maintenance treatment every 1 to 4 months is suggested because the pigment gradually declined toward baseline values during a 6-month follow-up. Although the authors performed 308-nm excimer light only once weekly for 6 times in our patient, she had a good response and rapid repigmentation within 3 months, and no recurrence of hypopigmentation was found at a 10-month follow-up. A possible hypothesis is that she had no genetic predisposition for vitiligo, and she had a darker Fitzpatrick skin type. Patients with Fitzpatrick skin Type III and above generally respond better to an excimer laser than those with lower skin types. To the best of our knowledge, this is the first case of corticosteroid-induced perilymphatic linear hypopigmentation treated with 308-nm excimer light.
