  • Efficacy of microneedling versus fractional Er:YAG laser in facial rejuvenation

    摘要: Background: Microneedling and fractional lasers have been used in facial rejuvenation with acceptable results and low adverse effects. Aims: To compare the efficacy of microneedling with fractional Er:YAG in facial skin rejuvenation. Patients/Methods: This study was planned as a split-face clinical trial. Volunteers were randomly allocated to receive three monthly treatments on each side of the face, one with fractional Er:YAG laser and one with microneedling. The assessments included investigating clinical outcomes by two blinded dermatologists accompanied by measuring skin biophysical characteristics including cutaneous resonance running time (CRRT) and transepidermal water loss (TEWL). Moreover, possible adverse effects, downtime, and patients' satisfaction were recorded at baseline, 1 month after each treatment, and 3 months after the last treatment session. The protocol was approved by the Iranian Registry of Clinical Trials (IRCT20160820029436N3). Results: Of the 32 selected volunteers, 24 subjects completed the study. The clinical assessment showed a significant improvement of the face appearance along with a significant reduction in dyschromia, and periorbital wrinkling (P-value < .05), with both procedures without any considerable difference between two methods. Moreover, the patients showed substantial satisfaction with both modalities with no statistically significant difference. Mean TEWL and CRRT values also decreased significantly in both groups with no considerable difference. The downtime was significantly shorter in the microneedling-treated side. There were no long-lasting or severe adverse effects after treatment with both methods. Conclusion: Microneedling and fractional Er:YAG laser have comparable efficacy in facial rejuvenation, but little downtime of the former makes it preferable for many patients.

  • Clinical and histopathological comparison of microneedling combined with platelets rich plasma versus Fractional Erbium-Doped Yttrium Aluminium Garnet (Er: YAG) Laser 2940a??nm in treatment of atrophic post traumatic scar: A randomized controlled study.

    摘要: Background: Different modalities have been tried for treating atrophic post traumatic scars. Objective: To compare the efficacy of microneedling combined with PRP with that of Fractional Er: YAG laser in treating post-traumatic atrophic scars. Methods: 45 patients with atrophic post traumatic scars were subdivided into three groups. Group I subjected to four sessions of skin microneedling followed by topical application of platelet rich plasma (PRP) immediately after each session. Group II subjected to the same protocol of treatment but for 6 sessions. Group III subjected to 4 sessions of Fractional Er: YAG laser at four weeks intervals for four sessions. Response to treatment was assessed clinically using the Quartile Grading Scale and histopathologicaly using H&E stain, trichrome and Orcein special stains. Results: All patients showed significant clinical improvement regardless of the treatment. There was a significant difference in clinical and histopathological improvement between group I and III, but there was no significant difference in improvement neither between group I and II nor between group II and III. Conclusion: microneedling could be as effective as fractional laser when combined with PRP and when used for more sessions.

  • Enhanced topical cutaneous delivery of indocyanine green after various pretreatment regimens: comparison of fractional CO2 laser, fractional Er:YAG laser, microneedling, and radiofrequency

    摘要: Different devices have been used to enhance topical drug delivery. Aim of this study was to compare the efficacy of different skin pretreatment regimens in topical drug delivery. In six ex vivo human abdominal skin samples, test regions were pretreated with fractional CO2 and Er:YAG laser (both 70 and 300 μm ablation depth, density of 5%), microneedling (500 μm needle length), fractional radiofrequency (ablation depth of ± 80–90 μm), and no pretreatment. The fluorescent agent indocyanine green (ICG) was applied. After 3 h, fluorescence intensity was measured at several depths using fluorescence photography. Significantly higher surface fluorescence intensities were found for pretreatment with fractional Er:YAG and CO2 laser and for microneedling vs. no pretreatment (p < 0.05), but not for radiofrequency vs. no pretreatment (p = 0.173). Fluorescence intensity was highest for the Er:YAG laser with 300 μm ablation depth (mean 38.89 arbitrary units; AU), followed by microneedling (33.02 AU) and CO2 laser with 300 μm ablation depth (26.25 AU). Pretreatment with both lasers with 300 μm ablation depth gave higher fluorescence intensity than with 70 μm ablation depth (Er:YAG laser, 21.65; CO2 laser, 18.50 AU). Mean fluorescence intensity for radiofrequency was 15.27 AU. Results were comparable at 200 and 400 μm depth in the skin. Pretreatment of the skin with fractional CO2 laser, fractional Er:YAG laser, and microneedling is effective for topical ICG delivery, while fractional radiofrequency is not. Deeper laser ablation results in improved ICG delivery. These findings may be relevant for the delivery of other drugs with comparable molecular properties.
