    摘要: Macular disorders, such as age-related macular degeneration (AMD) and diabetic macular edema (DME), have plagued humans for many years, yet the traditional recognition methods are often complicated, time-consuming and laborious. Therefore, this paper proposes an effective recombined residual convolutional neural network (CNN) with less computation to recognize AMD, DME and the normal of SD-OCT images. This paper adopts the18-layer Residual CNN as the basis and removes some of convolutional layers in the convolutional groups which extract low-level or high-level features, while add them to the middle part of the CNN, to enhance the ability of extracting mid-level features. This paper also adjusts the kernel size to change the visual receptive fields during convolution calculations. Experiments show that the recombined residual CNN with the kernel size of 3x3 shows better performance than the original one and recently proposed methods on the same dataset, with the highest overall accuracy up to 90%. Moreover, in terms of feature extraction complexity and time consumption, the recombined residual CNN is also more dominant.

  • Role of OCT Angiography in the Detection of Retinal Vascular and Macular Abnormalities in Subjects with Asteroid Hyalosis

    摘要: Purpose: To study the role of Optical coherence tomography (OCT) Angiography (OCTA) in detecting retinal vascular and macular abnormalities as compared to Fundus Fluorescein Angiography (FFA) in subjects with Asteroid hyalosis (AH). Methods: In a prospective study, patients with AH underwent OCTA and FFA. AH graded as Grade 1 in 8 (optic disc, second order vessels visible), Grade 2 in 4 (optic disc, first-order vessels visible), Grade 3 in 11 (hazy view of optic disc) and Grade 4 (no view of fundus) in 2 eyes. Results: A total of 20 patients (25 eyes) with AH were included. In comparison to FFA, OCTA was able to similarly detect Diabetic Retinopathy changes such as Neovascularization, microaneurysms, capillary dropout, and foveal avascular zone extent in vascular occlusion in all grades of AH. Conclusion: OCTA, a noninvasive imaging tool, could detect various retinal vascular and macular abnormalities in patients with all grades of AH in comparison to FFA.

  • Instant Stent-Accentuated Three-Dimensional Optical Coherence Tomography Guided Selection of Proper Distal Cell for Side Branch Dilatation in Bifurcation Stenting

    摘要: In the bifurcation stenting, the distal rewiring for the side branch postdilatation confirmed by two-dimensional modalities may not lead to favorable results in some cases. If there are two distal cells divided by the link bridging from the carina, the rewiring through the larger distal cell may be recommended for the side branch postdilatation. Detailed confirmation of the rewired cell by the intraprocedural instant stent-accentuated three-dimensional optical coherence tomography is important.

  • Clinical relevance of protruded retinal layers in minimum rim width measurement of the optic nerve head

    摘要: background/aims Optical coherence tomography (OCT) imaging of the optic nerve head minimum rim width (MRW) has recently been shown to sometimes contain components besides extended retinal nerve fibre layer (RNFL). This study was conducted to determine whether excluding these components, termed protruded retinal layers (PRLs), from MRW increases diagnostic accuracy for detecting glaucoma. Methods In this cross-sectional study, we included 123 patients with glaucoma and 123 normal age-similar controls with OCT imaging of the optic nerve head (24 radial scans) and RNFL (circle scan). When present, PRLs were manually segmented, and adjusted MRW measurements were computed. We compared diagnostic accuracy of adjusted versus unadjusted MRW measurement. We also determined whether adjusted MRW correlates better with RNFL thickness compared with unadjusted MRW. results The median (IQR) visual field mean deviation of patients and controls was ?4.4 (?10.3 to ?2.1) dB and 0.0 (?0.6 to 0.8) dB, respectively. In the 5904 individual B-scans, PRLs were identified less frequently in patients (448, 7.6%) compared with controls (728, 12.3%; p<0.01) and were present most frequently in the temporal sector of both groups. Areas under the receiver operating characteristic curves and sensitivity values at 95% specificity indicated that PRL adjustment did not improve diagnostic accuracy of MRW, globally or temporally. Furthermore, adjusting MRW for PRL did not improve its correlation with RNFL thickness in either group. Conclusion While layers besides the RNFL are sometimes included in OCT measurements of MRW, subtracting these layers does not impact clinical utility.

  • Large axial range frequency-domain optical low coherence interferometry

    摘要: In this paper, we propose a new experimental setup to extend the measurement range of frequency-domain low coherence interferometry (FD-LCI) from 3 mm to 2 cm. The FD-LCI can be extended to the technique of frequency domain optical coherence tomography (FD-OCT) by performing additional lateral scanning. This new and simple arrangement just attaches an extra interferometer before the spectrometer in a typical FD-LCI setup. With this configuration, it is possible to overcome the dynamic range limitation of the conventional technique. This allows us to measure the optical path difference of the sample of interest, which is outside the detection range of the spectrometer, using an extra interferometer as a tuner system. In addition, this tuning allows significant minimization of the effects produced by the fall-off. The accuracy of the new system is obtained by adding the FD-LCI and the tuning micrometric translator accuracies. In our experimental setup, this value is 20 μm.

  • Total Retinal Blood Flow in a Nonhuman Primate Optic Nerve Transection Model Using Dual-Beam Bidirectional Doppler FD-OCT and Microsphere Method

    摘要: PURPOSE. We validated noninvasive Doppler-optical coherence tomography (OCT) blood ?ow measurements against the terminal microsphere method in a surgical induced optic nerve transection nonhuman primate model. METHODS. In 6 nonhuman primates, total retinal blood ?ow (TRBF) was measured with a custom-built dual-beam bidirectional Doppler Fourier Domain (FD)-OCT. Peripapillary retinal nerve ?ber layer thickness (RNFLT) was measured by Spectralis spectral-domain (SD)-OCT. Measurements were performed every 10 to 15 days before and after unilateral optic nerve transection (ONT) until RNFLT was reduced by more than 40% from baseline. Before the animals were killed, TRBF was measured using the microsphere technique. RESULTS. A signi?cant correlation between all arterial and venous Doppler OCT TRBF measurements was found in ONT and contralateral control eyes (both P < 0.01, n ? 6). The Bland-Altman analysis showed a bias of 0.57 in the ONT group and 0.02 in the contralateral control group. Also, excellent agreement was observed between Doppler OCT and microsphere measurements (P < 0.01, r ? 0.976, bias ? 0.54). After ONT, TRBF and RNFLT decreased by (cid:2)51% 6 42% and (cid:2)44% 6 2% (n ? 5), respectively. In the contralateral control eyes, TRBF and RNFLT were unchanged. CONCLUSIONS. Very good accordance was found between TRBF measurements, obtained with dual-beam bidirectional Doppler FD-OCT and the microsphere method. It also was possible to monitor changes over time in TRBF after ONT with Doppler OCT. These ?ndings highlight the accuracy and potential of noninvasive Doppler OCT to provide valuable information for detecting early changes in ocular disease in future.

  • An update on inflammatory choroidal neovascularization: epidemiology, multimodal imaging, and management

    摘要: Inflammatory choroidal neovascular membranes are challenging to diagnose and manage. A number of uveitic entities may be complicated by the development of choroidal neovascularization leading to a decrease in central visual acuity. In conditions such as punctate inner choroidopathy, development of choroidal neovascularization is extremely common and must be suspected in all cases. On the other hand, in patients with conditions such as serpiginous choroiditis, and multifocal choroiditis, it may be difficult to differentiate between inflammatory choroiditis lesions and choroidal neovascularization. Multimodal imaging analysis, including the recently introduced technology of optical coherence tomography angiography, greatly aid in the diagnosis and management of inflammatory choroidal neovascularization. Management of these neovascular membranes consists of anti-vascular growth factor agents, with or without concomitant anti-inflammatory and/or corticosteroid therapy.

  • Hyperreflective Intraretinal Foci as an OCT Biomarker of Retinal Inflammation in Diabetic Macular Edema

    摘要: We read with great interest the article titled ‘Association Between Soluble CD14 in the Aqueous Humor and Hyperre?ective Foci on Optical Coherence Tomography in Patients with Diabetic Macular Edema’ by Lee et al. The authors investigated the expression of soluble CD14 (sCD14) in aqueous humor (AH) samples of patients with diabetic macular edema (DME), and the presence of hyperre?ective foci (HF) on spectral-domain optical coherence tomography (SD-OCT) in these eyes. The authors found both higher levels of sCD14, and increased number of HF in the inner retina compared to controls, and concluded that, since sCD14 is released by retinal microglia, HF might represent aggregates of activated microglial cells in DME eyes. The presence of and changes in OCT HF, also called hyperre?ective spots, have been previously described in detail in diabetic eyes and reported to depend on activated microglia. DME-affected eyes treated with intravitreal anti-VEGF and steroids have also been investigated, demonstrating a reduction of HF number with both treatments—con?rming HF as a retinal in?ammatory biomarker. To improve this observation, we also investigated diabetic eyes in a large population to identify HF-speci?c characteristics, a topic not suf?ciently addressed by Lee et al. In particular, we disagree with the statement that HF, as aggregates of retinal microglial cells, have the same re?ectivity as the retinal pigment epithelium (RPE), whereas it is more similar to that of the retinal nerve ?ber layer (RNFL). Other clinical characteristics (size < 30 lm, absence of back-shadowing, presence in both inner and outer retinal layers in DME) allow differentiation of HF, as an in?ammatory biomarker, from hard exudates, small intraretinal hemorrhages, microaneurysms, and tiny capillaries, which also appear as OCT hyperre?ective foci (spots). The correct identi?cation of in?ammation-driven HF from other forms of hyperre?ective material is relevant when using this new biomarker in the follow-up of any DME treatment. As consequence, any investigation of HF on OCT in diabetic eyes should be more precise to avoid inconsistent results.

  • Comparison of Anterior Segment Measurements between Scheimpflug Camera and New Module of Optical Coherence Tomography

    摘要: Purpose: We compared the anterior segment measurements using a Scheimpflug camera (Pentacam? [Oculus Inc., Wetzlar, Germany]) and a new anterior segment module of optical coherence tomography (Cirrus HD-OCT 5000? [Carl Zeiss Meditec, Dublin, CA, USA]). Methods: Anterior segment measurements were evaluated in 47 eyes of 26 patients who visited for the purpose of cataract surgery. Measurements of the central corneal thickness (CCT), anterior chamber depth (ACD), anterior chamber angle (ACA; nasal and temporal side), and angle to angle distance (ATA) were compared between the Pentacam? and the new anterior segment module of the Cirrus HD-OCT 5000?. Results: The mean CCTs measured by the Pentacam? and the new anterior segment module of the Cirrus HD-OCT 5000? were 561.68 ± 31.35 μm and 559.04 ± 36.43 μm, respectively. There was no statistically significant difference (p = 0.387) and high correlations in the results using the two methods (r = 0.824, p < 0.001). The mean ACD measurements using the Pentacam? and Cirrus HD-OCT 5000? were 2.70 ± 0.44 mm and 2.68 ± 0.42 mm, respectively. The measurements were strongly correlated (r = 0.981, p < 0.001) without a statistically significant difference (p = 0.091). The measurements of ACA (nasal and temporal side) and ATA did not differ significantly different between the two methods (p = 0.109, p = 0.153, p = 0.322, respectively). High correlations were observed between the two methods (r = 0.944, r = 0.951, r = 0.955, respectively; p < 0.001 in all groups). Conclusions: There was no significant difference between the CCT, ACD, ACA, and ATA measured by the Pentacam? and the new anterior segment module of the Cirrus HD-OCT 5000? in adult patients. However, there was a low degree of agreement for the CCT. The measurements of ACD, ACA, and ATA can therefore be considered interchangeable when used clinically.

  • Retinal vessel optical coherence tomography images for anemia screening

    摘要: Anemia is a disease that leads to low oxygen carrying capacity in the blood. Early detection of anemia is critical for the diagnosis and treatment of blood diseases. We find that retinal vessel optical coherence tomography (OCT) images of patients with anemia have abnormal performance because the internal material of the vessel absorbs light. In this study, an automatic anemia screening method based on retinal vessel OCT images is proposed. The method consists of seven steps, namely, denoising, region of interest (ROI) extraction, layer segmentation, vessel segmentation, feature extraction, feature dimensionality reduction, and classification. We propose gradient and threshold algorithm for ROI extraction and improve region growing algorithm based on adaptive seed point for vessel segmentation. We also conduct a statistical analysis of the correlation between hemoglobin concentration and intravascular brightness and vascular shadow in OCT images before feature extraction. Eighteen statistical features and 118 texture features are extracted for classification. This study is the first to use retinal vessel OCT images for anemia screening. Experimental results demonstrate the accuracy of the proposed method is 0.8358, which indicates that the method has clinical potential for anemia screening.
