  • A Randomized Controlled Trial Comparing Platelet-Rich Plasma, Low-Level Laser Therapy, and Complex Decongestive Physiotherapy in Patients with Lower Limb Lymphedema

    摘要: Background: Platelet-rich plasma (PRP) is an autologous concentrated preparation of platelets characterized by lymphangiogenetic and tissue-repairing effects. Although PRP has been safely used in many different fields, there is no clinical study regarding the use of PRP in lymphedema treatment in humans. We assessed the clinical outcomes of PRP in patients with lower extremity lymphedema (LEL) in a randomized controlled trial. Methods and Results: Patients with secondary LEL were randomly allocated to one of three groups consisting of treatment with PRP with complex decongestive physiotherapy (PRP+CDP group), low-level laser therapy with CDP (LLLT+CDP group), and only CDP (CDP group). Assessment of Lymphedema Quality of-Life Questionnaire (LYMQOL) for health-related quality of life, lower-extremity-circumference (LEC) for edema, tissue dielectric constant (TDC) for extremity volume, 6-minute walking test (6MWT) for functional capacity, and numeric rating scale (NRS) scoring for extremity fullness were evaluated both before and after treatment. Forty-five patients (68.8% female) with mean age 40.84 – 15.81 years were included in the study. Significant differences in LYMQOL, LEC, NRS, and TDC values both before and after treatment were found in all groups; however, there were no statistically significant difference in values between the three groups. In the PRP+CDP group, LYMQOL values had a larger effect size than the other two groups. Significant differences in 6MWT values both before and after treatment were found in PRP+CDP and LLLT+CDP groups; however, there was no statistically significant difference in the CDP group. Conclusion: This is the first clinical study to evaluate the usage of PRP in patients with secondary LEL. PRP might be an additional treatment option of lymphedema management; however, more clinical trials in humans are needed to yield more evidence in the usage of PRP in patients with lymphedema.

  • A prospective safety and effectiveness study using endovenous laser ablation with a 400-??m optical fiber for the treatment of pathologic perforator veins in patients with advanced venous disease (SeCure trial)

    摘要: Background: Treatment of pathologic perforator veins (PPVs) can shorten time to healing and reduce recurrence of ulcers in patients with advanced venous disease. Because of limited clinical evidence and device options, widespread adoption of PPV treatment is controversial. The objective of this study was to evaluate the safety and efficacy of endovenous laser therapy using a 400-mm optical fiber to treat PPVs. Methods: This study was a single-arm, prospective, seven-center, nonblinded clinical study examining patients with advanced skin changes or healed or active ulceration (Clinical, Etiology, Anatomy, and Pathophysiology clinical class C4b, C5, and C6). Patients received treatment with a 1470-nm laser. Procedural technical success and 10-day primary closure were evaluated. All device-related adverse events were reported. Follow-up of patients was continued for 12 months after initial ablation. Results: The primary PPV closure (at 10-day visit) rate was 76.9% (95% confidence interval, 70.3%-82.4%). Successful primary closure rates of 75.7%, 70.3%, 62.1%, 68.8%, and 71.3% of PPVs were achieved at 1 month, 3 months, 6 months, 9 months, and 12 months, respectively. Statistically significant improvements (P< .05) were seen in patients’ quality of life at 1 month, 3 months, 6 months, 9 months, and 12 months compared with screening. The percentage of patients with ulcers (22.9% at screening, 14.1% at 1 month, 13.7% at 3 months, 10.1% at 6 months, 12.3% at 9 months, and 11.1% at 12 months) displayed improvement during the course of the study. Tibial deep venous thrombosis and procedural pain were the only device-related adverse events observed. Conclusions: Endovenous laser therapy for PPV using the 400-mm optical fiber with the 1470-nm laser yielded safe and effective outcomes with no major adverse sequelae.

  • Surveillance With PET/CT and Liquid Biopsies of Stage I-III Lung Cancer Patients After Completion of Definitive Therapy: A Randomized Controlled Trial (SUPER)

    摘要: Despite increased focus on prevention as well as improved treatment possibilities, lung cancer remains among the most frequent and deadliest cancer diagnoses worldwide. Even lung cancer patients treated with curative intent have a high risk of relapse, leading to a dismal prognosis. More knowledge on the ef?cacy of surveillance with both current and new technologies as well as on the impact on patient treatment, quality of life, and survival are urgently needed. We therefore designed a randomized phase 3 trial. In one arm, every other computed tomography (CT) scan is replaced by positron emission tomography/CT, the other arm is the standard follow-up scheme with CT. The standard arm is identical to the current national Danish follow-up program. The primary endpoint is to compare the number of relapses treatable with curative intent in the 2 arms. We aim to include 750 patients over a 3-year period. Additionally, we will test the feasibility of noninvasive lung cancer diagnostics and surveillance in the form of circulating tumor DNA analysis. For this purpose, blood samples are collected before treatment and at each following control. The blood samples are stored in a biobank for later analysis and will not be used for guiding patient treatment decisions.

    摘要: Conversation and mastication are related to the quality of life. If one is healthy in a comfortable situation, the time length of the conversation and the mastication will be long. In addition, the time length of the conversation is an index to measure the sociality while that of the mastication is an index of health. Therefore, the measurement of the conversation and the mastication must be key factors of the monitoring in, for example, nursing homes for the elderly. A Doppler sensor is a device which enables us to realize non-contact sensing of the movement of a body. It means that there is no need to put electrodes on the body. We can naturally think about the non-contact sensing of the jaw movement caused by the conversation and the mastication. However, it is not easy to distinguish the conversation from the mastication. In this paper, we apply a signal processing scheme CARS for this problem and experimentally verify the possibility of the time length measurement for the conversation and the mastication.
