Selective Laser Sintering 3D Printing a?? An Overview of the Technology and Pharmaceutical Applications
摘要: Food and Drug Administration (FDA) has approved a drug product (Spritam?) and many medical devices manufactured by 3-dimensional printing (3DP) processes for human use. There is immense potential to print personalized medicines using 3DP. Many 3DP methods have been reported in the literature for pharmaceutical applications. However, selective laser sintering (SLS) printing has remained least explored for pharmaceutical applications. There are many advantages and challenges in adopting a SLS method for fabrication of personalized medicines. Solvent free nature, availability of FDA approved thermoplastic polymer/excipients (currently used in hot melt-extrusion process), minimal/no post-processing step, etc. are some of the advantages of the SLS printing process. Major challenges of the technology are requirement of at least one thermoplastic component in the formulation and thermal stability of drug and excipients. This review provides an overview of the SLS printing method, excipient requirements, process monitoring, quality defects, regulatory aspects and potential pharmaceutical applications.

Laser surgery in management of medication-related osteonecrosis of the jaws: a meta-analysis
摘要: Purpose The aim of this study was to perform a systematic review and meta-analysis to evaluate the efficacy of laser therapy on medication-related osteonecrosis of the jaw (MRONJ) treatment. Methods This study followed PRISMA standards, and an electronic search was performed on the PubMed/MEDLINE, Scopus, and Cochrane databases. Eighty-nine articles were found. After reading the manuscripts, 15 articles remained for the review. Three of them were selected for meta-analysis. Results Female gender was predominant (72.5%), and mean age was 66.5 years. Follow-up varied between 3 and 80 months, and the most used bisphosphonate was zoledronic acid (71.6%). Stage II of MRONJ was the most prevalent (68.9%), and the mandible was the most affected site (64.5%). Qualitative data showed that treatment with laser surgery (Er:YAG) was most effective regarding complete healing of the lesion (90%) compared with other treatments. Meta-analysis data showed that low-level laser therapy (LLLT) was more effective than medical treatment (P = 0.006), and surgical treatment was more effective than LLLT (P = 0.008). Conclusion Laser surgery was significantly superior to LLLT (p < 0.00001). Therefore, laser surgical therapy seems to be a great management strategy for MRONJ treatment from stage II. LLLT was shown to improve conservative management in earlier MRONJ stages.

Selective laser trabeculoplasty versus medication for open-angle glaucoma: systematic review and meta-analysis of randomised clinical trials
摘要: background The only widely accepted, effective treatment for open- angle glaucoma (OAG) is to reduce the intraocular pressure (IOP), with medical therapy being the typical first- line therapy. Notably, an alternative therapy is selective laser trabeculoplasty (SLT), which is safe and effective in lowering the IOP. Nonetheless, whether SLT could replace medication as the first- line therapy for OAG is still under debate. Methods Studies involving randomised controlled trials conducted before August 2019 that compared the efficacy of SLT- related and medication- only treatments for OAG were selected from PubMed, Embase, Cochrane Library and Web of Science. Grading of Recommendations, Assessment, Development and Evaluations (GRADE) methodology was employed to rate the quality of the body of evidence. results 1229 patients in eight trials were included. The overall results revealed no significant differences between SLT- related and medication- only treatments regarding the IOP reduction (mean difference (MD): 0.18, 95% CI ?0.72 to 1.07, p=0.70, I2=73%) and the success rate of IOP control (risk ratio: 1.02, 95% CI 0.99 to 1.04, p=0.74, I2=0%). The SLT- related therapy group required significantly fewer medications compared with the medication- only group (MD: ?1.06, 95% CI ?1.16 to ?0.96, p<0.0000, I2=5%). A quantitative analysis was not performed concerning adverse events and quality of life because of the limited data available. Conclusion SLT is safe and has a lower incidence of ocular side effects. SLT can be the choice of first- line therapy for OAG. However, clinicians should consider the cost- effectiveness, as well as the patient’s characteristics, before deciding on the therapeutic option.
