Plasmonic Microneedle Arrays for in situ Sensing with Surface-Enhanced Raman Spectroscopy (SERS)
摘要: Surface-enhanced Raman spectroscopy (SERS) is a sensitive, chemically specific, and short-time response probing method with significant potential in biomedical sensing. This paper reports the integration of SERS with microneedle arrays as a minimally invasive platform for chemical sensing, with a particular view towards sensing in interstitial fluid (ISF). Microneedle arrays were fabricated from a commercial polymeric adhesive and coated with plasmonically active gold nanorods that were functionalized with the pH-sensitive molecule 4-mercaptobenzoic acid. This sensor can quantitate pH over a range of 5 to 9 and can detect pH levels in an agar gel skin phantom and in human skin in situ. The sensor array is stable and mechanically robust in that it exhibits no loss in SERS activity after multiple punches through an agar gel skin phantom and human skin or after a month-long incubation in phosphate-buffered saline. This work is the first to integrate SERS-active nanoparticles with polymeric microneedle arrays and to demonstrate in situ sensing with this platform.

Sandwich-structure transferable free-form OLEDs for wearable and disposable skin wound photomedicine
摘要: Free-form optoelectronic devices can provide hyper-connectivity over space and time. However, most conformable optoelectronic devices can only be fabricated on flat polymeric materials using low-temperature processes, limiting their application and forms. This paper presents free-form optoelectronic devices that are not dependent on the shape or material. For medical applications, the transferable OLED (10 μm) is formed in a sandwich structure with an ultra-thin transferable barrier (4.8 μm). The results showed that the fabricated sandwich-structure transferable OLED (STOLED) exhibit the same high-efficiency performance on cylindrical-shaped materials and on materials such as textile and paper. Because the neutral axis is freely adjustable using the sandwich structure, the textile-based OLED achieved both folding reliability and washing reliability, as well as a long operating life (>150 h). When keratinocytes were irradiated with red STOLED light, cell proliferation and cell migration increased by 26 and 32%, respectively. In the skin equivalent model, the epidermis thickness was increased by 39%; additionally, in organ culture, not only was the skin area increased by 14%, but also, re-epithelialization was highly induced. Based on the results, the STOLED is expected to be applicable in various wearable and disposable photomedical devices.

Optimization of Laser Therapy Parameters in Dependence of Spot Size
摘要: Recently, the laser therapy for a treatment of the skin lesions of the vascular origin has gained a substantial development, and progress in the technical area. The vascular lasers, particularly the CANDELA Vbeam Perfecta, which is a part of this research has been equipped by a system of the dynamical cooling, called DCD leading to a higher patient’s comfort during the examination. In the most of the vascular lasers, it was allowed for settings of the pulse length up to 40 ms. By this way, the hematomas are reduced after the treatment, and mostly the skin is only getting red. For a reason of a complexity of the CANDELA Vbeam Perfecta settings, on a demand of the physicians, the user environment has been developed serving for an automatic parameters calculation of the laser therapy, and possibilities of alternative settings for same laser energy. The software environment has been designed in the MATLAB. The SW has been tested in the clinical conditions of the University Hospital of Ostrava.

Multiple Squamous Neoplasms Arising in a Red Tattoo After Laser Tattoo Removal
摘要: Tattoos are common around the world, although not without risk. Tattooing may result in several complications, including infections, foreign-body and sarcoidal granulomatous reactions, delayed-type hypersensitivity reactions, keloid formation, pseudolymphomas, pseudoepitheliomatous hyperplasia, as well as koebnerization and pathergy of associated pre-existing skin diseases. Tattoos may even potentiate cutaneous neoplasms including benign growths, such as seborrheic keratoses, and malignant neoplasms, such as nonmelanoma skin cancers (NMSCs) and malignant melanoma. Although the mechanism underlying neoplasia due to tattoos is poorly understood, it has been suggested that trauma from the procedure itself, the carcinogenic potential of the dyes, or chronic inflammation could play a role. In this study, we present a case of multiple NMSCs developing within the red pigmented area of a tattoo after repeated laser tattoo removal treatments.

Fractional Ablative Laser Therapy is an Effective Treatment for Hypertrophic Burn Scars
摘要: Objective: The aim of this study is to determine objective and subjective changes in mature hypertrophic burn scars treated with a fractional ablative carbon dioxide (CO2) laser. Background: Fractional CO2 laser treatment has been reported to improve burn scars, with increasing clinical use despite a paucity of controlled, prospective clinical studies using objective measures of improvement. Methods: A multicenter, site-controlled, prospective open-label study was conducted from 2013 to 2016. Objective and patient-reported outcome measures were documented at baseline, at each monthly laser treatment, and 6 months after treatment. Objective measurements employed were: mechanical skin torque to measure viscoelastic properties; ultrasonic imaging to measure scar thickness; and reflectometry to measure erythema and pigmentation. Subjective measures included health-related quality of life, patient and investigator scar assessment scales, and blinded scoring of before and after photographs. Subjects aged 11 years or older with hypertrophic burn scars were recruited. Each subject received 3 monthly treatment sessions with an ablative fractionated CO2 laser. Results: Twenty-nine subjects were enrolled, of whom 26 received at least 1 fractional CO2 laser treatment and 22 received 3 treatments. Mean age of those completing all 3 treatments was 28 years. Statistically significant objective improvements in elastic stretch (P < 0.01), elastic recovery (P < 0.01), extensibility (P < 0.01), and thickness (P < 0.01) were noted. Patient- and physician-reported scar appearance and pain/pruritus were significantly improved (P < 0.01). There was no regression of improvement for at least 6 months after treatment. Conclusions: Fractional ablative laser treatment provides significant, sustained improvement of elasticity, thickness, appearance, and symptoms of mature hypertrophic burn scars.

A regimen to minimize pain during blue light photodynamic therapy of actinic keratoses: Bilaterally controlled, randomized trial of simultaneous versus conventional illumination
摘要: Background: Blue light photodynamic therapy (PDT) is effective for actinic keratosis (AK), but many patients experience stinging pain during illumination. Objective: Compare a conventional regimen (1 h ALA preincubation, followed by blue light), to a new modified regimen in which blue light is started immediately after ALA application. Methods: A clinical trial with a bilaterally controlled, intrapatient study design was conducted in 23 patients. Topical 20% ALA was applied to the entire face and/or scalp. On one side of the body, blue light was started immediately and continued for either 30, 45, or 60 min (“simultaneous PDT”). On the contralateral side, blue light began 1 h after ALA application and lasted 1000 sec (“conventional PDT”). Pain was evaluated on a 0-10 scale. AK lesion counts were determined by clinical exam and photography. Results: All patients experienced significantly less pain during simultaneous illumination than during the conventional regimen. At 3 months post-treatment, lesion clearance was nearly identical on the two sides, as determined by statistical testing of non-inferiority ± 15% margin. Limitations: Although bilaterally controlled, the study was relatively small. Additional studies are recommended. Conclusion: The modified PDT regimen is essentially painless, yet provides treatment efficacy similar to a conventional regimen.

761 The impact of 308-nm excimer laser on human keratinocytes of organ cultured human skin
摘要: Evidence that UVB-generated microvesicle particles are involved in acute skin inflammation. Microvesicle particles (MVP) are small membrane bound particles released from cells that have been demonstrated to act as signal transporters between cells via their abilities to transport bioactive molecules. Previously our laboratory has reported that environmental injuries such as ultraviolet B radiation (UVB) and thermal burn injury can generate MVP release via a novel pathway involving the Platelet-activating factor (PAF) receptor and acid sphingomyelinase (aSMase) in epithelial cell lines and human skin. Our current studies demonstrate that MVPs released from keratinocytes in vitro and mice in vivo in response to UVB (UVB-MVP) are dependent upon PAF receptor and aSMase. Moreover, these UVB-MVP contain high levels of PAF. Finally, since PAF is involved in acute skin inflammation in response to UVB, we tested whether UVB-MVP could be contributory through their abilities to transport PAF. Murine studies reveal that blocking aSMase inhibits acute, PAF-dependent inflammatory responses. These findings suggest that MVP provide a mechanism by which the metabolically labile bioactive lipid PAF leaves the epidermis in response to acute environmental stressors like UVB. Moreover, targeting MVP could provide a novel strategy to block UVB-induced skin inflammation.

Laser-assisted MAL-PDT associated with acoustic pressure wave ultrasound with short incubation time for field cancerization treatment: a left-right comparison
摘要: Background: Laser-assisted MAL-PDT has been reported to increase the effectiveness of conventional PDT. Nonetheless, clinical effects of this association when reducing MAL is poorly discussed. Furthermore, the association of acoustic pressure wave ultrasound with laser-assisted MAL-PDT with short incubation time for field cancerization had not been reported before. Objectives: To compare clinical effects of ablative fractional laser-assisted MAL-PDT associated with acoustic pressure wave ultrasound (IMPACT US) with 1-hour field cancerization on the forearms, as well as the impact on safety and tolerability. Methods: Fifteen patients with 638 AK (grade I-III) with field cancerized-skin on the forearms were enrolled in this left-right trial. Two protocols were randomly chosen. One side was treated with conventional MAL-PDT, whereas the other with laser-assisted MAL-PDT associated with acoustic pressure wave ultrasound with 1-hour incubation time. Actinic keratoses were quantitively measured, and the other signs of sun-damaged skin, like pigmentation and texture, in field cancerized skin were qualitatively evaluated before and after six months. Side effects were assessed subjectively during the procedure and one week after. Results: All patients completed the study. At six months after treatment, both protocols reduced the number of AK (72%; CO2 + PDT, and 65%; MAL-PDT). The difference between these two protocols was not statistically significant (p = 0.77). The improvement of pigmentation and texture of field cancerized skin was more significant on the side treated with laser-assisted MAL-PDT associated with acoustic pressure wave ultrasound. Both protocols were well tolerated and without significant difference in adverse events. Conclusion: Laser-assisted MAL-PDT using CO2 laser and acoustic pressure wave ultrasound with short incubation time of 1 hour was as effective as conventional MAL-PDT for field-cancerized skin with actinic keratosis in forearms with better cosmetic outcome.

752 Clinical effects of LED blue light on skin pigmentation and carotenoid depletion
摘要: The growing evidence that visible light - and especially blue light - contributes to detrimental effects of sun exposure to skin, is becoming a subject of controversial debate nowadays. In this study we aimed to assess blue light-induced pigmentation and carotenoid depletion in vivo. Furthermore, we wanted to show a protective effect of SPF50 sunscreen formulations in a double-blind, placebo-controlled trial. The UVA/B filter system included the microfine particles titanium dioxide and methylene bis-benzotriazolyl tetramethylbutylphenol that extend effectiveness into the blue light range. Comparisons between repetitive irradiation with LED blue light (450 ± 20 nm) with 45 J/cm2 over 5 consecutive days after daily product application (2 mg/cm2) and irradiation only once with 135 J/cm2 after product application over 6 days were performed on the back of the volunteers. The extent of pigmentation and carotenoid depletion was investigated with the assessment of the skin color (ITA°) by chromameter measurements and the in vivo carotenoid concentration via multiple spatially resolved reflectance spectroscopy, respectively. Assessments were performed 5 hours, 24 hours, 1 week and 2 weeks after the last irradiation. Fourteen subjects terminated the study. Both, the single high-dose and the repetitive exposure resulted in comparable, immediate (≤5h) increase of skin pigmentation and carotenoid degradation (p<0.001). Skin pigmentation and carotenoid depletion attenuated on all test conditions already one day after the last irradiation. Both effects were still evident one and two weeks after the last irradiation. The application of the cosmetic formulations including UV pigments that scatter in the blue light range significantly reduced pigmentation (p<0.001) right after irradiation that might be considered for holistic light protection beyond UV.

Use of 3-Dimensional Imaging in Submental Fat Reduction Following Hyperthermic Laser Lipolysis
摘要: In a randomized controlled study, patients with a wide variety of hypopigmented scars, and striae alba had mean final pigment correction rates of approximately 100% by colorimetric analysis after 9 treatments of 308-nm excimer laser biweekly. However, maintenance treatment every 1 to 4 months is suggested because the pigment gradually declined toward baseline values during a 6-month follow-up. Although the authors performed 308-nm excimer light only once weekly for 6 times in our patient, she had a good response and rapid repigmentation within 3 months, and no recurrence of hypopigmentation was found at a 10-month follow-up. A possible hypothesis is that she had no genetic predisposition for vitiligo, and she had a darker Fitzpatrick skin type. Patients with Fitzpatrick skin Type III and above generally respond better to an excimer laser than those with lower skin types. To the best of our knowledge, this is the first case of corticosteroid-induced perilymphatic linear hypopigmentation treated with 308-nm excimer light.
