Patients show impaired calf, but maintain forearm, oxygenation and microcirculatory function during laparoscopy, regardless of the anaesthetic technique. Reduction in post-ischaemic recovery with high-dose spinal analgesia is explained by its sympatholytic effects: number of perfused capillaries is increased, leading to a haemodynamically more favourable state.

1. The study could not wait 20 min to assess the maximum extent of the block, as this might lead to prolonged (or eventually cancelled) surgery schedules. 2. Generalisations to other spinal solutions and doses should be made with caution.
1:Experimental Design and Method Selection:
Patients undergoing elective laparoscopic gynaecological surgery were randomised to receive general anaesthesia alone or in combination with high-dose or low-dose spinal analgesia. Near-infrared spectroscopy was used to measure microcirculatory effects over forearm and calf muscles utilising a 3-min ischemic challenge.

102 patients were enrolled, with standard monitoring including electrocardiography, pulse oximetry, and noninvasive blood pressure measurements.

Near-infrared spectroscopy sensors (SomaSensor SAFB-SM; Covidien Ilc, Massachusetts, USA), manual sphygmomanometer (DS55; Welch Allyn, Jungingen, Germany), manual pressure cuff (5255; Rudolf Riester, Jungingen, Germany).
4:Experimental Procedures and Operational Workflow:
Vascular occlusion tests (VOTs) were performed at three measurement points: before general anaesthesia induction, 5 min after tracheal intubation, and 15 min after pneumoperitoneum commencement.
5:Data Analysis Methods:
Statistical analysis was performed using SPSS Statistics 20 and graphs were designed in Prism 8. Distributions of data were tested for normality with pairwise Shapiro–Wilk test and the values of skew and kurtosis.
