To evaluate the degree of microvascular impairment in diabetic retinopathy using multifractal and lacunarity analyses and to compare the diagnostic ability between traditional Euclidean measures (fovea avascular zone area and vessel density) and fractal geometric features.

Multifractal and lacunarity analyses can be potentially valuable tools for assessment of microvascular impairments in diabetic retinopathy. Multifractal geometric parameters exhibit a better discriminatory performance than Euclidean measures, particularly for detection of the early stages of diabetic retinopathy.

First, this was a retrospective study with a limited number of participants, and DR patients with other ocular pathologies that might interfere with retinal vasculature were excluded from the study. Second, DME is very common in eyes with advanced DR, and we found that automated segmentation of the DCP layer was not always accurate in eyes with DME. Third, the scan quality of the OCTA images was relatively lower in DR patients than in normal controls because of the relatively poor fixation or vision in these patients.
