To describe different approaches to equip biomaterials with antimicrobial properties for use in ophthalmology to prevent infections associated with implants.

Infections associated with implants increase morbidity and costs, and with the loss of efficacy of conventional therapies, alternative treatments like antimicrobial biomaterials are needed. Preclinical studies support their efficacy, but due to the multifactorial nature of infections, further multidisciplinary research is required to develop biomaterials with real clinical application.

The infectious process is multifactorial, influenced by variables related to the pathogen, host, and environment, requiring additional multidisciplinary studies for a greater understanding and development of clinically applicable biomaterials. Some approaches have limited antibacterial spectrum, potential toxicity, or adverse effects, and many are still in the development stage with no clinical applications yet.
