研究目的
To evaluate the long-term results of transmyocardial laser revascularization (TMLR) using a CO2 laser in combination with intramyocardial injection of autologous bone marrow stem cells (ABMSC) as an isolated procedure in patients with end-stage coronary artery disease.
研究成果
TMLR in combination with intramyocardial injection of ABMSC is a safe and effective method for treating certain patients with end-stage coronary artery disease, providing a statistically significant antianginal effect and improving quality of life. Strict patient selection is essential for achieving positive outcomes.
研究不足
The study's limitations include the small number of patients (20) and the specific selection criteria, which may limit the generalizability of the results. Additionally, the long-term effects and mechanisms of TMLR and ABMSC injection require further investigation.
1:Experimental Design and Method Selection:
The study included 20 patients with end-stage coronary artery disease who underwent TMLR surgery in combination with intramyocardial injection of ABMSC. The Minnesota Living with Heart Failure Questionnaire (MLHFQ) and the Seattle Angina Questionnaire (SAQ) were used to assess quality of life.
2:Sample Selection and Data Sources:
Patients were selected based on diffuse nature of coronary artery disease, involving the distal bed, with no technical possibility of direct myocardial revascularization and no effect of optimal antianginal therapy.
3:List of Experimental Equipment and Materials:
A high-power CO2 laser 'Cardiolaser' was used for the procedure.
4:Experimental Procedures and Operational Workflow:
The operation was performed under general endotracheal anesthesia with access from left-sided anterior-lateral thoracotomy. Laser perforations were made in the myocardium, followed by injection of ABMSC.
5:Data Analysis Methods:
Statistical analysis was performed using parametric and nonparametric tests, with data presented as M ± SD for normally distributed data or Me (Q1; Q3) for non-normally distributed data.
