Management of a laryngeal venous malformation with Nd:YAG laser and bleomycin sclerotherapy
摘要: Literature describing neodymium:yttrium-aluminum-garnet (Nd:YAG) photocoagulation and sclerotherapy for laryngeal venous malformations (VMs) is sparse. Here we present a case in which an extensive laryngeal VM in a 28-year-old female was managed through a combination of four serial Nd:YAG laser photocoagulation sessions and four bleomycin injections over the course of 2 years. The treatment plan resulted in noticeable lesion ablation, mucosalization, and significant improvement in symptoms. To our knowledge, this case is the first instance of bleomycin injection specifically into a laryngeal VM reported in the English medical literature.

Effect of Q‐switched Nd‐YAG laser on the clinical, pigmentary and immunological markers in the patients with lichen planus pigmentosus: A pilot study
摘要: The persistent discolouration in lichen planus pigmentosus is secondary to persistent melanophages in superficial dermis in these patients. We evaluated the effect of Q-switched Nd-YAG laser on the clinical, pigmentary and immunological markers in the patients with lichen planus pigmentosus. Nine females with a clinical diagnosis of lichen planus pigmentosus were included in the study. After six sessions of laser with toning protocol, performed over a representative area of 5x5 cm2 at 2-weekly intervals, the mean clinical improvement as per the physician assessment was 25.7% (10-40%). There was no significant reduction in melanin and erythema index. On expression analysis using reverse transcriptase polymerase chain reaction, the reduction in tyrosinase (p=0.03) was statistically significant, though, the reduction in other pigment and immunological markers was not significant. The immunohistochemistry quantification data of corresponding proteins also did not show any significant difference. Post inflammatory hypopigmentation was noted in one patient. Q-switched Nd-YAG laser toning protocol resulted in modest clinical and histological improvement in patients of lichen planus pigmentosus.

The evaluation of long‐pulsed Nd:YAG laser efficacy and side effects in the treatment of cutaneous vessels on the face and legs
摘要: Background: Long‐pulsed Nd:YAG lasers have been used for the treatment of cutaneous vascular lesions on face and leg veins. Objective: To report our experience with long‐pulsed Nd:YAG lasers for the treatment cutaneous vessels on the face and legs. Materials and Methods: A total of 145 patients with cutaneous vessels on the face and the legs were included in this study. All patients received two treatment sessions, first at the beginning and second after four weeks. Patients were evaluated three times: first at the beginning of the therapy, and second and third after the four weekly follow‐ups. The laser settings of each treatment session were recorded. The physician's assessment of treatment efficiency was taken by comparing gross photograph and clinical evaluation. The grade of improvement, as the percent of reduction in the severity of veins after laser treatment, was determined as follows: <25% improvement (mild); 25%–50% improvement (moderate); 51%–75% improvement (good); and >75% improvement (excellent). The side effects were also recorded. Results: Of the 145 patients who completed the study, 63.6% of patients had vessels on the face, and 36.4% had them on the legs. After the first session, 82.8% of patients have good improvement (51%–75%). After the second session, 78.6% of patients have excellent improvement (>75%). The clinical improvement after the second treatment session was significantly higher than the first treatment session (P ? .05). There were no statistical differences between the treatment areas (P ? .05). The most common side effect was erythema in 12.4% (P ? .05). Conclusion: The Nd:YAG laser is an effective and safe treatment option for the treatment of cutaneous vessels on the face and the legs.

Laser for vascular anomalies: successful outcomes in children
摘要: Vascular anomalies can cause both emotional and physical distress to patients, particularly children. The paediatric laser service at Great Ormond Street Hospital (GOSH) treats a range of dermatological conditions including a variety of vascular anomalies, excess hair growth and disfiguring scars. The laser team at GOSH has 25 years of experience in treating a wide variety of paediatric dermatological conditions using various laser therapies. With over 600 new referrals for laser therapy and over 1000 laser procedures each year the GOSH laser team has vast amounts of experience with both common and rare conditions. Excellent clinical outcomes continue to be delivered, and new treatment therapies are constantly being developed to treat more recalcitrant lesions. The adverse effect rates experienced by the GOSH laser patients have been decreasing over the past two decades, reaching the low rate of 0.8% per treated patients per year. This remarkable achievement has been continuously improved by integrating specific and standardized laser protocols for each patient treated, to ensure efficacious and safe laser treatment delivery. Treating vascular anomalies with laser therapy creates significant positive results among the paediatric population, thus laser therapy at GOSH makes a significant impact upon children’s lives with both rare and common vascular anomalies.

Laser treatment for onychomycosis
摘要: Background: Laser systems are a common treatment choice for onychomycosis. They exert their effects on inhibiting the growth of the fungus by selective photothermolysis but efficacy is dependent on the specific type of apparatus used. To systematically review the available published literature on the curative effects and safety of laser treatment for onychomycosis. Methods: Databases including PubMed, web of science, China National Knowledge Internet (CNKI), WanFang Database and VIP were searched systematically to identify relevant articles published up to July 2018. Potentially relevant articles were sourced, assessed against eligibility criteria by 2 researchers independently and data were extracted from included studies. A meta-analysis was performed using R software. Results: Thirty-five articles involving 1723 patients and 4278 infected nails were included. Meta-analysis of data extracted from these studies revealed that: the overall mycological cure rate was 63.0% (95%CI 0.53-0.73); the mycological cure rate associated with the 1064-nm Nd: YAG laser was 63.0% (95%CI 0.51-0.74); and that of CO2 lasers was 74.0% (95%CI 0.37-0.98). The published data indicate that laser treatment is relatively safe, but can cause tolerable pain and occasionally lead to bleeding after treatment. Conclusion: Laser treatment of onychomycosis is effective and safe. The cumulative cure rate of laser treatment was significantly higher for CO2 lasers than other types of laser. Laser practitioners should be made aware of potential adverse effects such as pain and bleeding.

Experience of 138 Transurethral Urethrotomy With Holmium:YAG Laser
摘要: Introduction: Many valid option modalities are available for the management of urethral stricture disease (USD), such as internal urethrotomy which has the success rates of 33%–60%. The aim of this study was to assess the outcome of holmium: YAG (Ho: YAG) laser urethrotomy (HLU). Methods: One hundred thirty-eight patients with urethral stricture with the mean age of 48±3.03 years old treated by HLU from March 2011 to August 2017. The main purpose of this investigation was to evaluate mean operation time, stricture recurrence rate and post-operation Qmax and complications of transurethral HLU. Results: The most common cause of USD was trauma in 82 (59.4%) patients. Mean laser operation time, mean hospital stay and mean postoperative duration of catheterization were 23.08 ± 9.1 minutes, 19.02 ± 10.7 hours and 10.3 ± 1.05 days respectively. The mean Qmax was 8.3 ± 2.07 mL/s before surgery and 16 ±3.1 mL/s afterward. At the end of 12 months follow-up, a total of 37 (26.8%) patients developed recurrence of the stricture. Patients with posterior, longer urethral strictures and previous history of interventions have more recurrence rate of the stricture. Conclusion: HLU is minimally invasive and seems to be an effective and safe management option for primary, short, urethral strictures. The hospital stay is remarkably short and complications are negligible.

Mid-IR Laser Generating Ultrasound in a Polyetheretherketone Polymer
摘要: We demonstrate laser ultrasonic generation in polyetheretherketone (PEEK). A middle infrared ZnGeP2 optical parametric oscillator (ZGP-OPO) pumped by a Q-switched Ho:YAG laser is employed as the ultrasonic excitation source. The ZGP-OPO has a spectral range of 3.2–3.4 μm. At an output wavelength of 3.4 μm, the maximum average output power of ZGP-OPO is 3.05 W with a pulse width of 24.3 ns, corresponding to a peak power of approximately 127.5 kW. The ultrasound is generated by the laser converted from 3.2 to 3.4 μm in the PEEK composite. The maximum ultrasonic signal amplitude in PEEK is 33 mV under the condition of thermoelastic excitation at 3.4 μm. Ablation occurs in the CPRF sample when the energy fluence is over 122.45 mJ/cm2. PEEK has a stronger absorption at 3.4 μm and laser-ultrasound generation is influenced by the wavelength of the laser.

Q-PTP is an optimized technology of 1064-nm Q-switched neodymium-doped yttrium aluminum garnet laser in the laser therapy of melasma: A prospective split-face study
摘要: Quickly-pulse-to-pulse (Q-PTP) is the latest dual pulse mode Q-switched neodymium-doped yttrium aluminum garnet [QS Nd:YAG (QSNY)] laser technology that improves efficacy and minimizes side effects. In this study, the safety, efficacy, and advantages of Q-PTP and single-pulse laser mode of 1064-nm QSNY in the treatment of melasma were compared. Twelve healthy women were enrolled in this split-face study. Q-PTP and single-pulse laser mode of 1064-nm QSNY were applied to the treatment side and the control side, respectively. Physician's global assessment, patients' self-assessment and the modified melasma area and severity index (mMASI) scores were used to quantify pigmentation changes. Side effects were recorded. Mean mMASI scores were decreased significantly at the 4th and 12th-week follow-up visit compared to the baseline. Melasma lesion clearance was more than 50% in 58% of patients compared with the baseline. However, both sides of the same patient showed similar therapeutic effect. Minor pain experience and slighter skin erythema reaction were reported in Q-PTP laser mode treatment side compared with the control. No serious complications were found in any of the patients. Q-PTP laser mode of 1064-nm QSNY is an optimal laser therapy in the treatment of melasma with greater safety and superiority.

Comparison of Nd:YAG laser (532 nm green) vs diode laser (810 nm) photocoagulation in the treatment of retinopathy of prematurity: an evaluation in terms of complications
摘要: Purpose To compare the anterior and posterior segment complications of diode (810 nm) laser photocoagulation (LPC) and Nd:YAG (532 nm green) LPC in the treatment of retinopathy of prematurity (ROP). Patients and methods The 84 eyes of 43 patients treated with diode LPC (group 1) and 58 eyes of 31 patients treated with Nd:YAG LPC (group 2) for ROP in our clinic were enrolled in the study. Medical records of all patients were investigated retrospectively. The patients in each group were examined in terms of birth weights, gestational weeks, stage of retinopathy, number of lasers pots, laser parameters, and anterior and posterior complications of LPC. Results The mean birth weeks of group 1 patients were postmenstrual 27.7 ± 2.5 (23–33), while the mean birth weights were 1006.0 ± 334.5 (540–1980) grams. The mean birth weeks of group 2 patients were postmenstrual 27.4 ± 2.6 (23–33), while the mean birth weights were 1073.8 ± 329.2 (480–1720) grams. The mean numbers of laser spots were 1036.0 ± 515.2 (430–2410) in group 1 per eye, while the mean numbers of laser spots were 1085.4 ± 526.0 (445–2530) in group 2 per eye (p ≥ 0.05). Additional laser application was performed in four eyes (4.8%) in group 1 and four eyes (6.9%) in group 2. Four eyes (4.8%) treated with diode LPC and one eye (1.7%) treated with Nd:YAG laser developed retinal detachment. Two eyes of a patient (3.4%) applied Nd:YAG LPC developed cataract. Conclusion Cataract may develop when Nd:YAG laser is used; however, posterior segment complications may be more likely to appear with the use of diode laser in these cases.

Synthesis of gold nanoparticles by pulsed laser-assisted reduction of aqueous gold precursor
摘要: In this paper, colloidal gold nanoparticles (GNPs) were synthesized by Nd:YAG (λ = 1064 nm) pulsed laser-assisted reduction (PLAR) of aqueous [AuCl4]?. In this process, during laser ablation of a Mo target in DI water, a [AuCl4]? precursor solution was being added dropwise in situ. UV–Vis absorption spectra showed distinct plasmonic absorption bands at 209 and 530 nm corresponding to Mo nanoparticles (NPs) and GNPs, respectively. Furthermore, no significant conversion for [AuCl4]? was found when it was added to aged Mo NPs or when it was irradiated in the lack of the Mo target. Using transmission electron microscope (TEM) images, it was found that precursor concentration and laser-ablation time can affect the population or size of GNPs. In PLAR with a small amount of KI solution in water, the GNPs size increases to ~ 50 nm via an iodine-induced particle fusion mechanism. The reduction mechanism was discussed based on metastable species intercation, producing the cavitation bubble during laser ablation.
